Referral Admission RN
Visiting
Nurse & Hospice Home is seeking an RN to receive patient related
telephone calls and patient referrals. This position requires the ability
to recognize the needs and emergencies of hospice and palliative patients.
This position participates in on-call rotation. Must have an RN license to
practice in the state of Indiana,
excellent organizational skills and a commitment to superior customer
service. EOE.
CNA
Visiting
Nurse & Hospice Home is seeking a full-time CNA. This person will be a
key member of the interdisciplinary team that provides care for palliative
and hospice patients and their families. The person in this position
provides total patient care in the home or a nursing facility and
participates in on-call rotation. Must
have a current CNA license, be CPR certified, have a valid Indiana driver’s
license and current automobile insurance.
RN Case Managers
Visiting
Nurse & Hospice Home is seeking two RN Case Managers. This position
entails home hospice care and coordination with the interdisciplinary team
and the ability to recognize the needs and emergencies of hospice and
palliative patients. This position provides patient care in the home or a
nursing facility and participates in on-call rotation. Must have a current
RN license to practice in Indiana, be CPR
certified, have a valid Indiana
driver’s license and current automobile insurance.
